Philosophy

Continuing Care Nova Scotia

Continuing Care Nova Scotia conducts its’ activities in accordance with the following principles:

  • Continuing Care Nova Scotia fosters client independence, empowerment and client/family involvement in care.

  • Continuing Care Nova Scotia supports the individual and helps to sustain the natural support system by supplementing, not replacing the assistance available from the family, informal caregivers and other community resources.

  • Continuing Care Nova Scotia provides high quality, effective, efficient and safe services to individuals at home.

  • Continuing Care Nova Scotia services are allocated in a consistent, equitable manner based on individual client need as determined through standardized assessment.

  • Continuing Care Nova Scotia services are provided by the most appropriate, cost efficient care provider as long as that care is with the provider’s scope of practice, knowledge and skill.

  • Continuing Care Nova Scotia works closely with existing community resources. Where services are available in the community and are appropriate and accessible to the individual, Continuing Care Nova Scotia does not duplicate those services.

  • Continuing Care Nova Scotia recognizes that care in a facility is appropriate when the resources of the individual, the family, the community and the program cannot adequately sustain the individual in the home.

  • Continuing Care Nova Scotia works in partnership with the community and the health care system and continually seeks new ways to respond to identify community needs.

  • Case management functions including intake, assessment, resource allocation and service authorization re separated from direct service delivery to ensure objectivity in care planning.
